File of Life & Yellow Dot

The “Yellow Dot” and “File of Life” Programs are a free service provided to Mount Laurel Residents of all ages, with an emphasis on our senior citizens.

File of Life

The File of Life Program is designed to assist Mount Laurel Residents and our local emergency first responders in the event of a medical emergency at the participant’s home. The File of Life is a mini medical history posted on the outside of one’s refrigerator. The File of Life card enables first responders to obtain quick medical history when a patient is unable to offer one and helps to eliminate the risk of a patient in a frightened state, forgetting to mention something important.

The entire packet is held with a magnet to the outside of the refrigerator. The card, which is kept in a red plastic pocket labeled File of Life lists the following information:

  • Patient Name

  • Emergency Contact

  • Medical Insurance Information

  • Health Problems

  • Medications and Dosages

  • Allergies

  • Recent Surgeries

  • Religion

  • Health Care Proxy

Yellow Dot

The Yellow Dot Program is designed to assist Mount Laurel Residents and our local emergency first responders in the event of an automobile accident or other medical emergency involving the participant’s vehicle. The program can help save lives during an emergency by reducing the time it takes to gather important information at a time when accident victims may be unable to communicate for themselves.

  • Name

  • Photo

  • Emergency Contact Information
